Mitral valve disease

With the impressive average of 150 mitral repairs annually for the last 16 years (possibly a world record, even more impressive given the population of Portugal), the Coimbra team led by Antunes reflects on six cases of circumflex artery injury, mostly in non-right dominant anatomy as expected.

Using a data set from 682 US hospitals during a 10-year period, the relationship of hospital procedure volume for AVR and MVR to adjusted mortality rate was evaluated.  Using terciles of volume to classify hospitals according to quality resulted in misclassification of 305 hospitals.  Hospital volume has only a modest association with quality as judg

Trends in the use of bioprosthetic versus mechanical valves for AVR and MVR were assessed using a California-based surgical population from 1996-2013.  During this period the use of bioprosthetic devices increased from 12% to 52% for AVR and from 17% to 54% for MVR.  Bioprostheses were associated with substantially increased 15-year mortality in youn

The authors present a step-by-step description of mitral valve annuloplasty ring implantation that takes into account not only the surgical technique, including positioning of annular stitches and correct sizing,  but also mitral annulus anatomy and its close relationship with surrounding structures.

Duerr and colleagues describe the experimental development of a mitral anuloplasty ring, which should facilitate later transcatheter valve-in-ring implantation. Two accompanying videos demonstrate valve expansion and implantation within the ring.

The authors have concluded from a meta-analysis of over 45,000 patients, mitral regurgitation in the presence of LV dysfunction (either ischaemic cardiomyopathy or idiopathic causes) is, not surprisingly, associated with reduced survival. Whether this is due to the underlying myopathic process or the MR per se is not clear.

Neil Moat of The Royal Brompton Hospital in London, United Kingdom, reviews the technologies currently being developed for transcatheter mitral valve repair and replacement.
