This video demonstrates the excision of subvalvular ring in a 34-year-old woman who was five feet three inches tall and weighed 275 pounds with minimal aortic regurgitation 

A femoral venous cannula and direct aortic cannulation was performed.  The aorta was cross-clamped and opened.   

The aortic valve appeared abnormal, and the U-shaped subvalvular ring was identified and described. Three nerve root retractors were utilized to protect the aortic valve leaflets.   

The subvalvular ring was excised, and muscle was removed along with the ring.  The ring was located close to the conduction system, the medial fibrous trigone, and the anterior mitral valve leaflet. The video concludes with a demonstration of the complex excision, followed by closing the aorta.  

The bypass time was 33 minutes, and the cross-clamp time was 24 minutes.
