Induction chemoradiotherapy is a relative contraindication to VATS lobectomy due to oncological concerns. This video demonstrates a VATS lower lobectomy and radical systematic lymphadenectomy in a 53-year-old male who recurred locally following induction chemoradiotherapy.

The authors demonstrate the steps for three-patch repair of congenital supravalvar aortic stenosis in a 5 month old.

This video shows in detail skeletonization of the internal mammary artery using the Harmonic hook blade, with footage contrasting the pedicle technique and traditional skeletonization with the cautery tip as a dissector.

In this video, the authors demonstrate their current technique and the pitfalls encountered when performing bioprosthetic pulmonary valve replacement in the adult congenital patient.

The Ross procedure is a valuable alternative for the surgical treatment of aortic valve disease in adolescent and young adult. However, neo-aortic root dilatation resulting in valve regurgitation is not uncommon. This video demonstrates the authors' technical modification that prevents this complication.

The lack of an ideal valve substitute in the congenitally affected aortic valve has raised the interest in aortic valvuloplasty by leaflet extensions. Several techniques using native or xenopericardium have been described for aortic valve repair, however, the ideal material for leaflet extensions still remains controversial.

Cardiac surgeons largely have converted to reparative procedures for tricuspid valve disease. Techniques that have been successful for mitral repair have facilitated the transition to repair. This video illustrates current repair methods in 8 patients with a variety of tricuspid valve disorders.

The aortic translocation operation for d-TGV, VSD, and PS was first performed in June, 1983. Since then, the senior author has been directly involved in 24 such operations with one early death.

Demonstrated in the video is a novel concept for facilitating extensive neochordal repair in mitral regurgitation based on the paradigm “respect rather than resect”. Both leaflet tissue and motion are respected. The leaflet is folded and the neochordae is tied, maintaining free margin and annulus edge-to-edge.
