Professor Pinto raises, in this editorial, a number of important points with regards to heart failure. Starting from a succinct definition of classification by ejection fraction, proceeds to analyse the notion of varied aetiologies of heart failure. Additionally, the editorial offers a robust critique of a small trial of SGLT2: the manuscript is easily read and will probably be quoted often, especially for its granular discussion on natriuretic peptides as markers of cardiac impairment in relation to BMI.
