July 10, 2019
Tirone David's experience on his operation, with interesting data on the durability of the re-implantation.

Thoracic stentgrafts are stiffer than the aorta. To understand how this mismatch might affect the left ventricle, van Bakel and colleagues quantified the left ventricular remodeling after thoracic endovascular aortic repair (TEVAR) in 8 patients.

Colleagues from five heart centers performed a study on the incidence and factors associated with acute stroke following type A repair using the Society of Thoracic Surgeons Adult Cardiac Surgery Database (STS ACSD).

Thoracoabdominal aortic aneurysm (TAAA) repair began in Houston, Texas, with  pioneer surgeons such as Drs Michael E. DeBakey, Denton A. Cooley, and E. Stanley Crawford. Their early attempts to repair TAAA were complicated by risk of renal and spinal cord ischemia and difficulty in reattaching the branching vessels of the thoracoabdominal aorta.

As the procedure began, doctors found that one lung was stuck to the chest bone forming bullae, which are like blisters and often occur with COPD. The surgeons punctured one of the blisters, causing an air leak. So that the patient wouldn't have trouble breathing, the anesthesiologist increased the amount of oxygen.

The NEJM published the long-term results of the OVER study of open repair (OR) versus endovascular aneurysm repair (EVAR) of abdominal aortic aneurysm (AAA) on May 30.

An interesting metanalysis, especially: The ad hoc results of off-pump surgery. The mean number of silent brain infarcts per patient was significantly higher in the transcatheter aortic valve implantation group (4.58 ± 2.09) compared with the aortic valve replacement group (2.16 ± 1.62, P=0.03).

The overarching, free to read under terms, of the TAVR mini-compendium of this week: the most complex question remains "what is a reasonable expectation of a stakeholder on outcomes of aortic stenosis?"
