The authors review the growing suspicion that fluoroquinolone antibiotic (FQ) use may be related to the occurrence of thoracic aortic aneurysms and acute aortic dissection. The proposed mechanism is through an increase in matrix metalloproteinases (MMPs), which degrade collagen, a precursor to aortic injury.

Reporting of postmarket outcomes for the Sapien 3 and Mitraclip devices frequently misclassified patient deaths as device malfunction or other outcomes. Misclassification for Sapien 3 involved 17.5% of patient deaths, and involved 24.7% of patient deaths for Mitraclip.

Marc R. Moon of the Washington University School of Medicine in St Louis, Missouri, USA, presents a discussion on the surgical repair options for aortic regurgitation.

The authors investigated cusp size in patients undergoing aortic valve repair for valve regurgitation. They found that most patients with small cusps have annulus cusp mismatch. They suggested that small cusp size is not a contraindication in aortic valve repair and recommended to secure annulus by plications to correct annulus cusp mismatch.

In this systemic review and meta-analysis, the authors compared outcomes of valve-sparing root replacement vs. the Bentall procedure in patients with acute aortic dissection type A. Valve-sparing root replacement was associated with lower risk of early and late mortality compared with the Bentall procedure.

Open distal anastomosis in the frozen elephant trunk operation is usually performed in aortic arch zone 2 or 3. The authors compared outcomes in patients who underwent zone 2 versus 3 open distal anastomosis. They found that proximallization of the anastomosis simplifies the arch replacement, reducing the visceral ischaemia time.

Thomas Modine of the Regional University Hospital of Lille in France presents a discussion and demonstration of his modified valve sparing technique for aortic valve replacement that employs the 3f Enable valve.

Concomitant aortic root enlargement at the time of surgical aortic valve replacement is an option to avoid patient-prosthesis mismatch. In this single-center study, 4,210 patients underwent aortic valve replacement, of which 171 had concomitant aortic root enlargement.
