The anterior cervicothoracic approach was used to remove a desmoid tumor of the thoracic outlet in a 70-year-old male. The chest wall was reconstructed with titanium bars and a polytetrafluoroethylene-patch. The clavicle was fixed using a titanium clip. The postoperative period was uneventful.

This video demonstrates the ligation of the thoracic duct by a VATS approach. The patient suffered damage to the duct in the neck resulting in neck swelling and made local repair of the duct impossible. Left internal thoracic artery (ITA) grafting to the left anterior descending artery has long become the gold standard. There is a trend for bilateral ITA grafting, commonly to left-sided arteries. The video shows ITA skeletonization, with low harvesting time, and minimal chest and graft thermal trauma.
