Valve Disease

Destruction of the intervalvular fibrosa (IVF) in infective endocarditis makes radical debridement followed by reconstruction of the IVF as the best treatment available for these patients. Authors describe their techniques in dealing with these extremely challenging cases, and show their results.

The incidence of coronary artery disease among patients with severe aortic stenosis and candidates for TAVI ranges from 40 to 75%. In the absence of randomized trials, the management of these patients remains to be established. In this paper, the authors offer a good overview of the subject and propose an algorithm for treatment.

In this study the authors use data from 1432 patients included in the German TAVI registry to identify variables which predict poor outcome in patients left with more than mild aortic regurgitation (AR) following TAVI.  The Medtronic Core Valve prosthesis had been used in the great majority of the procedures.

A step-by-step guide for performing an aortic root replacement with the Freestyle Aortic Root. Safeguards and pitfalls are reviewed.

The authors of this paper prospectively studied stroke after aortic valve replacement (AVR) in patients with aortic stenosis (AS) and >65years old. Neurological status was assessed pre- and postoperatively assessed by neurologists, postoperatively by MRI and strokes were evaluated according to the National Institutes of Health Stroke Sscale.

This multi-centre registry study assessed peri-operative and long-term outcomes in patients who underwent isolated surgical aortic valve replacement (AVR) who had previous undergone coronary artery bypass graft surgery (CABG). The study included 113 patients who underwent AVR after previous CABG across seven centres between 2000-2013.

The aims of this study were, in patients with MVP undergoing MV repair, to describe the prevalence and characteristics of MAC, to correlate MAC with clinical risk factors, coronary involvement, and aortic valve disease, and to describe prevalence, site, and extension of MAC in fibroelastic deficiency (FED) versus Barlow's disease (BD) and correlate M

Filmed at the 2014 STS Annual Meeting in Orlando, Florida, this roundtable discussion focuses on the treatment of mitral valve disease. Vinod Thourani moderates the discussion with Gorav Ailawadi, Vinay Badhwar, and Michael Argenziano.

The PORTICO trial has opened to evaluate the Portico Transcatheter Aortic Valve System.  This prospective multicenter trial involving up to 40 sites will randomize pts to Portico vs another TAVR valve in the US.

The authors present results of surgery in 12 pts for hypertrophic cardiomyopathy extending to the papillary muscles.  Repair was performed through a detached anterior mitral leaflet, which was subsequently repaired.  The intraventricular end-diastolic gradient was decreased by 80% postop, and mitral regurgitation that was present preop was eliminated
