Pts undergoing aortic root replacement who moderate MR preoperatively were evaluated for outcomes after mitral repair vs no repair. Mitral repair was associated with longer crossclamp times, and postoperative renal failure. Root replacement improved MR without mitral repair, but mitral repair further improved the degree of regurgitation.

Results of bicuspid valve reconstruction for endocarditis have been suboptimal due to dehiscence of pericardial patches and other causes. This video illustrates complete replacement of destroyed bicuspid leaflets, using glutaraldehyde-fixed autologous pericardial leaflets.

Mini-sternotomy is being increasingly adopted for surgery of the aortic valve and ascending aorta. This adds greater complexity when compared to a standard median sternotomy approach. This video describes technical modifications that allow the surgeon to overcome these challenges in a case requiring deep hypothermic circulatory arrest (DHCA), aortic valve, and hemiarch repair.
