This video shows a 53-year-old male patient with severe COPD. His computed angiography showed a Crawford Type I thoracoabdominal aneurysm. His right kidney was atrophic, and the aneurysm was extending just proximal to the left renal artery.

Mini-sternotomy is being increasingly adopted for surgery of the aortic valve and ascending aorta. This adds greater complexity when compared to a standard median sternotomy approach. This video describes technical modifications that allow the surgeon to overcome these challenges in a case requiring deep hypothermic circulatory arrest (DHCA), aortic valve, and hemiarch repair.

Tricuspid valve regurgitation is not uncommon in patients with hypoplastic left heart syndrome and has been shown to be a significant factor affecting the success of staged surgical palliation in these patients. Left internal thoracic artery (ITA) grafting to the left anterior descending artery has long become the gold standard. There is a trend for bilateral ITA grafting, commonly to left-sided arteries. The video shows ITA skeletonization, with low harvesting time, and minimal chest and graft thermal trauma.
