This case demonstrates the repair of a membranous ventricular septal defect (VSD) several weeks post transcatheter aortic valve replacement (TAVR) with an unappreciated root rupture. The patient required VSD patching as well as root enlargement using a modified Manouguian technique to place a 25 mm tissue valve. She also had a significant right coronary lesion, which was addressed with a bypass graft. Additionally, a left ventricular vent was placed through the left atrial appendage, which was later clipped, which the the authors consider routine. This video highlights the importance of being comfortable with TAVR explants and root enlargement techniques, both of which are becoming a routine part of cardiac surgery due to the rise of TAVR procedures in low-risk patients.
