Tumors invading the tracheobronchial angle or the carina represent a challenge due to the complexity of airway reconstruction and management. Carinal resection, a complex and aggressive procedure for the treatment of these tumors, may be a safe approach for highly selected patients, in experienced centers with acceptable morbidity and mortality and good long-term survival.

A right robotic thymectomy is described for the treatment of myasthenia gravis and thymoma.  Early results are comparable to sternotomy and VATS approaches.  A novel technique using ICG for contralateral phrenic nerve identification is also described.

Induction chemoradiotherapy is a relative contraindication to VATS lobectomy due to oncological concerns. This video demonstrates a VATS lower lobectomy and radical systematic lymphadenectomy in a 53-year-old male who recurred locally following induction chemoradiotherapy.

For purposes of this survey, respondents were asked to assume they had recommended surgical therapy for a 63 year old man who had been diagnosed with a distal esophageal adenocarcinoma, clinically staged as T2N0 via EUS, CT, and PET. His general health was good. He had no prior abdominal or thoracic surgery, and he had no important comorbidities.

A young female with a single metastasis at the level of the upper portion of the sternum underwent surgical excision of the sternum followed by sternal allograft implantation. The use of a sternal allograft provides excellent functional and cosmetic results.
