Aortic valve disease

November 7, 2019
Aortic annulus size and shape were analyzed in computed tomography angiography after a reimplantation procedure in patients with bicuspid and tricuspid aortic valves. Prior to surgery, elliptic shape was observed in tricuspid valves and more circular shape in bicuspid valves.

Data from the PARTNER trials were assessed regarding post-procedural prosthetic valve endocarditis (PVE) after transcatheter aortic valve replacement and surgical aortic valve replacement. The timing and rates of PVE were similar between the groups, and they had a similar high mortality.

In this intraoperative video, the authors demonstrate the principles of the BASILICA technique in an instance where redo-surgical aortic valve replacement was the appropriate management strategy.

The authors review the growing suspicion that fluoroquinolone antibiotic (FQ) use may be related to the occurrence of thoracic aortic aneurysms and acute aortic dissection. The proposed mechanism is through an increase in matrix metalloproteinases (MMPs), which degrade collagen, a precursor to aortic injury.

Filmed at the STS Structural Heart Symposium in Chicago, Illinois, David Fullerton moderates a discussion on surgical training for transcatheter aortic valve replacement.

Reporting of postmarket outcomes for the Sapien 3 and Mitraclip devices frequently misclassified patient deaths as device malfunction or other outcomes. Misclassification for Sapien 3 involved 17.5% of patient deaths, and involved 24.7% of patient deaths for Mitraclip.

Marc R. Moon of the Washington University School of Medicine in St Louis, Missouri, USA, presents a discussion on the surgical repair options for aortic regurgitation.
October 8, 2019
The authors investigated cusp size in patients undergoing aortic valve repair for valve regurgitation. They found that most patients with small cusps have annulus cusp mismatch. They suggested that small cusp size is not a contraindication in aortic valve repair and recommended to secure annulus by plications to correct annulus cusp mismatch.

In this systemic review and meta-analysis, the authors compared outcomes of valve-sparing root replacement vs. the Bentall procedure in patients with acute aortic dissection type A. Valve-sparing root replacement was associated with lower risk of early and late mortality compared with the Bentall procedure.
