Aortic valve disease

December 30, 2014
This is a retrospective review of 260 patients undergoing TAVI, looking at the relationship between aortic valve calcium score and post procedural paravalvular leakage. The results suggest that the amount of calcification significantly impacts the occurrence, the degree and localization of paravalvular leakage after TAVI.

This experimental video shows a total reconstruction of the aortic valve with autologous pericardium using the author’s method of patch tailoring.

In this manuscript, the authors report on a retrospective study looking at the progression of aortic stenosis (AS) in 1558 patients with mild, moderate and severe AS and preserved left ventricular function. They conclude that the progression of severity depends significantly on the severity of the AS at baseline.

Andrew Goodwin of James Cook University Hospital, Middlesbrough, United Kingdom, discusses the MAVRIC trial, which is focused on the minimally-invasive manubrium-limited AVR procedure.

This video illustrates bicuspid annular stabilization during repair using a geometric annuloplasty ring that is currently under investigation in Europe (NCT02071849).

This manuscript reports on the findings of a meta-analysis of retrospective observational studies comparing clinical outcomes at 1-year in patients treated by either transfemoral (TF) or transapical (TA) transcatheter aortic valve implantation for sever aortic stenosis.
December 7, 2014
In this retrospective, multicentre study, the authors evaluate clinical outcomes in 139 patients with bicuspid aortic valves undergoing transcatheter aortic valve implantation (TAVI) with either a self or a balloon-expandable prosthesis.
December 3, 2014
In this video, the authors present their technique for aortic root replacement using a freestyle aortic root prosthesis and a Dacron graft, thus achieving a "Biological Bentall."

This study examined the effect of the introduction of TAVR on overall AVR rates in the US using the STS and STS/ACC registries.  From 2008 to 2013, AVR rates increased at hospitals performing TAVR by 69%, including a 22% increase in surgical AVR; the latter increase was primarily in low- and moderate-risk patients.  In contrast, non-TAVR hospital AVR

141,905 isolated primary surgical aortic valve replacements (SAVR) were stratified by STS risk of mortality.   SAVR outcomes were superior to those by the predicted risk models.
